Our program pairs trained community volunteers with seniors for regular, one-on-one walks in familiar, accessible environments. This innovative model addresses physical inactivity and social isolation—two key social determinants of health. Every participant receives a personalized walking plan following a safety assessment conducted by a licensed physical therapist. Walk times vary based on individual ability and range from 10 minutes to over one hour. Since 2021, we’ve facilitated over 5,000 walks for more than 200 seniors in Charleston, SC, supported by over 250 trained and active volunteers. Our structured, data-informed approach allows us to measure outcomes and ensure each participant receives consistent, quality support. A dedicated full-time Program Coordinator manages implementation.
